Musical Theater Auditions 2012 Teacher Entry Form



Checklist

  • All students listed are students to whom I personally give regular individual instruction.
  • I understand that if my students enter the competition, I am required to fulfill judging responsibilities.
  • I have read the audition rules and understand the repertoire requirements.
  • I have read and understood the Copyright Policy as set forth by The National
    Association of Teachers of Singing. This certifies that my students and their pianists will
    conform to the law.
  • I have had each student verify the information on the entry form and understand that if the information submitted is found to be incorrect, the student will be disqualified. Student's signature is not required.
  • I have checked with each accompanist to verify that they are not playing for more than a total of TEN STUDENTS. The accompanist may play for either High School or for College, Graduate, or for Adult students. (Please note the change for previous years).
  • I have collected all student fees and have submitted payment to PayPal on the GHNATS website for the total amount.

DEADLINE – Entry form must be completed online by Thursday, January 5, 2012.

Please list accompanists names, addresses, phone numbers, and e-mail addresses. (Those playing  for 10 students will be sent an  individualized schedule.)
